have this entry from yesterday
1 sportsillustrated.cnn.com (blocked by category) Cancel 468
This domain is no longer blocked. You can block this domain or re-block this entire category.
1 day.. 468 requests, and it claims to be no longer blocked, yet Sportsillustrated is not in my whitelist yet. even though it does appear to be popping up as a category that for most of year doesn't fit.. (Lingerie/Bikini)
468 seems like a large number of requests for a day here, as we only have 10 users on a day when all employees are here, and we only had half that probaly here yesterday.. -
Yeah it is tagged Lingerie/Bikini: http://domain.opendns.com/sportsillustrated.cnn.com
Also, the alias sportsillustrated.cnn.com is real cnnsi.com, but this is not yet tagged, and this may cause the strange message about "no longer blocked".
So if you visit these domains, are they blocked or not for you?
And yes, 468 is a proud number. Just by chance: using FireFox 3.5.x or Chrome/Iron with DNS prefetching enabled somehow in your network? If yes, disable DNS prefetching, and you should be good to get rid of this phenomenon.Thankful People: maintenance -

Funny thing is that for the 4th domain on the list of all requests for the 4th, right behind google, akamai and FB. firefox prefetching would still end up having all the requests go to opendns when Opendns is set as forwarders on our Microsoft DNS Server?

"all the requests go to opendns when Opendns is set as forwarders on our Microsoft DNS Server?"
Maybe improve the caching of the DNS server, if still possible at all? Whatever, this would not make much sense, as the TTL for sportsillustrated.cnn.com is just up to 15 minutes...
DNS caching on the workstations may help a bit as well. But most interesting thing would be to know what visited sites have so many or less links to this site so that it is DNS-prefetched so often.

(Hmm, this site seems to link back primarily to itself..., and maybe from CNN, of course.)
